Physicals
All physicals must be electronically uploaded to Healthy Roster even if one is on file with the school nurse. All students must establish a Healthy Roster account by completing the JotForm located on the school's website under Athletics/Sportsmedicine/Healthy Roster. A physical dated after May 19,2024 is required to be uploaded into this profile before they will be allowed to try out for a team, along with filling out and signing the Acknowledgement and Assumption of Risk and Emergency Contact and Travel Permission forms located under the documents tab of the athlete’s profile. Video tutorials, resources, and explanation of Healthy Roster required documents/features can be found on the sports medicine page of our school website: https://www.msjnet.edu/athletics/sports-medicine
If your insurance only allows a physical every two years, or you are not due for a new physical, we accept Patient First or Minute Clinic physicals. This will satisfy the requirements for participation in athletics.
